PERMISSION SUBJECT
This consent form has been prepared as an annex and an integral part of your existing employment contract, concerning the processing, storage, and sharing of "Personal Data" as regulated by the Protection of Personal Data Law No. 6698. "Personal Data" within the scope of this Consent Form and Law No. 6698 will mean all kinds of up-to-date information, including your information in the job application form and your personnel file, from your employment within the scope of the employment contract. This information can be obtained in written, verbal, audio, video or electronic media.
Our company has the title of "Data Controller" as defined in Law No. 6698. As the data controller, we state that Personal Data will be processed, recorded, stored, preserved, updated, and transferred when necessary. We inform you that any transaction on Personal Data by our company, Group Companies, affiliates, consultants, shareholders, or solution partners will be considered as personal data processing.
Our company processes your personal data to fulfill its obligations within the scope of all relevant national and international legislation, especially the Labor Law No. 4857, the General Health and Social Insurance Law No. 5510, and the Law on Occupational Health and Safety No. 6331. Your biometric data is recorded and processed by the systems used for the continuity and follow-up of employees.
Your personal data can be transferred to companies affiliated to the Group of Companies, our partner companies, domestic/foreign/international, public/private institutions and organizations, and other 3rd parties from which our company receives services or cooperates. We inform you that your Personal Data must be stored in accordance with all relevant legal regulations during the continuation of your employment contract and will be destroyed at the end of the legal periods.
By applying to our company, in accordance with Article 11 of Law No. 6698, you have the right to:
- Learn whether your personal data is processed, and if so, request information about it.
- Learn the purpose of processing and whether it is used in accordance with this purpose.
- Know whether the said data is transferred to third parties in the country or abroad.
- Request the correction of your personal data if it is incomplete or incorrectly processed.
- Request the deletion or destruction of your personal data within the framework of the conditions stipulated in Article 7 of the law.
- Object in case of a result against you due to the analysis of your personal data exclusively by automated systems.
- Demand compensation for damages if you suffer damage due to unlawful processing.

Within the framework of the explanations made above, I accept that I have read and understood this information and consent form and that I have the following statement received in this way.
In accordance with the Law on the Protection of Personal Data No. 6698, I give my explicit consent to the processing of my personal and/or sensitive personal data. This includes obtaining, recording, storing, changing, updating, periodically checking, rearranging, classifying, and keeping it for the period required for the purpose for which they are processed or stipulated in the relevant law. In case of legal or service-related actual requirements, I consent to sharing this data with public institutions and organizations that your company works with or is legally obliged to, and/or 3rd party service providers residing in Turkey or abroad, including supplier companies, your company and/or your company's partners, and companies under the umbrella of HIS Group of Companies, and transferring them abroad with my explicit consent, without any hesitation on the subject.
